Your first visit: what to do and what to expect

Your first visit to a facility like Michiana Hematology Oncology can seem a bit intimidating. We understand your anxiety and that you will have many questions. We will do everything in our power to put you at ease. There are also some steps that you can take to make your visit simpler and easier.

Please plan to arrive for your appointment 15-30 minutes before your scheduled time. This will allow ample time to fill out new patient forms. Most of these forms are provided here so you are welcome to download them and complete them in advance.

Also, please be sure to bring the following:

• Your medical insurance card
• Your prescription insurance card
• A current, valid picture identification
• A list of any medications you are currently taking and the dosages
• The name of your personal physician and any other physicians who may be involved in your care

At Michiana Hematology Oncology, our physicians and staff understand your time is as valuable as ours so we make every effort to assure that we will be available promptly at your appointment time. However, other patients occasionally require special or emergency attention which can cause unplanned and unavoidable delays. We ask your patience in these circumstances.
